This module is based on RAP4(03)- Surface Repair Using Form-and-Pour Techniques. The document is intended as a voluntary field guide for the owner, design professional, and concrete repair contractor interested in performing concrete surface repairs using form-and-pour techniques. The document was developed by ACI Committee E706 and approved by the ACI Educational Activities Committee.
